Before mailing in your application, please make sure you have included the following:

• Food Vendor Application and Food Vendor Agreement
• Electrical/Fire Safety Worksheet
• Application to Conduct a Temporary Event Food Facility sheet
• Food Booth Worksheet
• Certificate of Insurance naming Boulder Creek Events and the City of Boulder as additionally insured
• Copy of State of Colorado Food Service License
• Appropriate Space Fee = $1400 or $1700
• Health Department Fee = $30
• Electricity Fee = $75 or $200
• Cleaning Deposit = $100 (Separate Check)
• Discount (pre-Dec. 1, 2009) = -$50
• Late Fee (Post-March 1) = $50
